I don't have that phrasing under VoiceOver, Audio in settings if no headphones are connected. Under VoiceOver, Audio I have: Sounds and Haptics, Audio Ducking, which is off. Auto Select Speaker in Call, on. Send to HDMI, off. That's it. If I plug in headphones, then I have, Sound channels.
So, either your earphone jack has an issue and makes the phone think it has headphones connected, or the Bluetooth thinks there are headphones connected.
