Richard is correct, I have been using iOS devices sincde 2009 and Voiceover has always alerted at 20% and 10% although you don't get the alert if the phone is locked. For example, if you get the 20% alert, then play an audio book or music and lock the phone, you will not get the 10% alert, eventually the phone will just turn off when it gets to 0%. Apparently in iOS 14 I read that with more functionality in the Shortcuts app you could set, for example, an alert at 30% and even set it up so that you don't just get the option to enable low power mode, but you can set it up so it automatically turns on low power mode. There is no alert at 5% unlocked or not and nothing you can do in Settings will change or prevent the 20% and 10% alerts.
