I must be missing something, but I can use the roter and VoiceOver to navigate around a page by character, word and line.
David Andrews has a question mark at the end of the statement, so I don't know if he stating that Kindle has never been good at navigation or if he's asking a question about it. The ability to navigate around a book is also dependent on the book itself and this may not have anything to do with accessibility and the screen reader you're using.
