Hi. The Apple pencil is 99% useless with voiceover. The only thing it allows you to do is draw letters on the screen for handwriting. As far as the sketches go, no, they cannot be interacted with while voiceover is enabled. In fact, you can't even draw on the screen without voiceover being disabled.
