Apps are not actually backed up when you do an iTunes backup and neither is music. Those are synced and the information as to which apps you have on your phone and where they are in terms of homescreen position or location in folders and so on is part of the backup. If you have to do a restore and you have synced with iTunes and done a backup, then the apps are synced back to your phone from iTunes. If you back up to iCloud the same is true except that instead of your apps syncing from iTunes they are redownloaded from the app store.
