Hi Wendy, First of all, you do have to make sure you have the correct input selected on your TV because if you don't, then Voiceover may give you a prompt to turn it on and you would never know. Below is a link to an Apple support article about how to turn on Voiceover for the first time:
https://support.apple.com/en-mu/HT202149 If you are not sure you have selected the correct input, you might need a sighted person to check this and to practice with you as to how you can reliable change from, for example, HDMI 1 to HDMI 2.
