This has all the indicators of being a simple volume problem. Have her remove 
any headphones, then do a two-finger swipe up on her screen, so VoiceOver will 
read the whole thing. While it reads, have her hit the volume up buttons until 
she hears speech.

Given that the speaker works, and that VoiceOver is clearly not muted or 
disabled, the only conclusion is that the main volume is down. Remember that 
iOS uses independent volumes - you can blast your internal speaker, but then 
have headphones turned way down, and both volumes are saved. To make it more 
confusing, the volume used during phone rings or alarms is different even from 
the other two, which is why she is hearing VoiceOver when an alarm goes off.
