Check for an alias list that contains multiple paths to Maildirs where one (or more) of the Maildirs(accounts) do not exist.
qmail-local see's the missing Maildir as a temporary error, stops delivery to the list and schedules it for delivery again. And it will run into the same time on each subsequent delivery. THis has the result of deliverying "multiple copies" to the first users in the alias list.
