Just edit the qmail-mrtg.cfg as Ken says in the INSTALL file.

Change the lines that read

Target[smtpad]: `/usr/local/bin/qmailmrtg7 a /var/log/smtp`

to read

Target[smtpad]: `/usr/local/sbin/qmailmrtg7 a /var/log/qmail/qmail-smtpd` 
or whatever your directories are

and do so for the rest of them.
Worked fine for me.
