I kinda did an overhaul to get this working on my server.  The thing
with smtp-auth is that it is NOT required.  The smtp-auth feature only
comes into play when the mail client requests it.  So if you can try
relaying mail from a network that is not in your tcp.smtp file then it
should deny relaying.  If you kick in the smtp-auth then you should be
able to relay from that machine.
 
I have not found any logging for smtp-auth.  However if you enable
logging in the vckpw module then it will show up in the maillog.  I have
never setup the smtp-auth using xinetd or inetd either.
 
When I was testing my smtp-auth setup I renamed the patched qmail-smtpd
to qmail-smtpd-auth and then spawned a tcpserver on an unreserved port
so that I could play with it for awhile without having it in production.
 
>From what I gather it looks like your xinetd configuration looks OK.  I
saw that you are running the qmail-smtpd as the vpopmail user so I don't
see a reason why it would be unable to authenticate.  That also shows me
that you really don't need to set your vchkpw module setuid ("chmod a+s
vchkpw" is not required)
