Hello Arun: The gr-doa OOT was tested with UHD 3.10.1.0 and GNU Radio 3.7.10.1, but it should work with the latest releases, UHD 3.13.0.2 and GNU Radio 3.7.13.4.
It uses the default FPGA image that comes with whichever version of UHD that you're using. It does not require any custom or special FPGA image.
