I'm using Eclipse 3.4 + T5.1 + Run Jetty Run Eclipse plugin (includes Jetty 6.1.6) and haven't seen a 404 unless I mistyped something by hand. I tend to leave my app running a few hours to a couple days before I change something that the live class reloading doesn't pick up. Pretty solid combination for me so far. And yes, live class reloading is great.
mrg PS. BTW, this is on OS X using Safari (mainly). I'm not sure if it could be a browser issue tripping you up (although that seems unlikely).
