tried to run a message through "spamassassin -D" ?
that should give you debug/timing info.

How long does it take to get SMTP greeting message when you start
"/usr/sbin/sendmail -bs" as a non root user?

Hitting PYZOR_CHECK is scary.

not at all. well, it MAY cause some delay but the default pyzor timeout is
3.5 seconds
