Is it possible plesk is set up to only add the SA headers above some specified SA threshold? Amavis has a $sa_tag_level_deflt to specify this. (Default is 0, saw somebody with it set to 2.) So maybe your problem isn't the lack of inserting headers, but just SA configuration failing to classify the email as spam?
