Are you getting your email relayed through messagelabs? That IP has a rank of MED. And if you're getting your mail relayed through them, that IP should be listed in your trusted_networks, which would result in 216.166.12.31 (out001.collaborationhost.net) being looked up in DNSWL, which has a rank of NONE.
