simply do as much filtering as you can on the backup - smtp-level
blacklists, SA, antivirus, helo checks, they were all mentioned last few
days here.

I would encourage you using DCC (either directly or through SA) and
optionally delay delivering to primary MX if you can set it up (courier has
such option, qmail doesn't afaik). 

While there is BadMX plugin that intended to get mail delivered through
secondary MX when the primary MX is available (detects short delay between
backup and primary), I wasn't able to get it working yet (only a few hits
for forwarded mail).

Delaying such mail for a while will help you against early recipient problem
- the mail will be more easily catched if the checksum will appear in
blacklists, checksum services etc...
