Obviously my scoring is a little higher than yours and I catch it but there's not a lot to go on.

Spamhaus catches it in 2 of it's lists (PBL and XBL). I would really recommend implementing zen.spmauaus.org at the MTA (smtp) level as this will block much of this junk before it ever gets near SA.
