It is now working. I forgot to patch qmail-scanner to have --sa support. It is
now doing quarantine.

it is a qmail-scanner-queue.pl issue and requires delicate
config changes
also, because of that, we changed the clamav config to the
spamassassin clamav plugin way as well and stopped it in the above
qmail-scanner-queue.pl
