2 possible problems:

1) the user_prefs file over-rode your rewrite_header, disabling it.

2) you're using an integration tool that doesn't use SpamAssassin's
markups and merely uses SA to generate a hit list (ie: Qmail-Scanner
with fast spamassassin, MailScanner or Mimedefang.)
