what does "having problems" means exactly? did they pay someone to kill

no, you're not listed in "reasonable" DNSBLs. so the "insurance
providers" may be using their own lists and/or checks, or they may be
using trendmickey lists (previously MAPS RBL). note that they may not
like your domain name (they may think "family care" is a spam domain).
if you are their customer, then you are well placed to ask them.
