I'd also recommend MIMEDefang for integrating SpamAssassin into sendmail. It's a milter, like amavisd-new.
We've been using it for several years on our servers. It's very customizable -- basically if you can write something in Perl, you can do it in MD.
The authors also have a commercial product based on MIMEDefang, Can-It, which might be worth looking into.
