My only other thoughts are - maybe you may need to upgrade exim to a more recent version to keep the ability to talk to spamassassin working (probably not though...). Also have you got any memory softlimits set for exim / spamassassin or anything else ? In the past I've had issues with memory limits being hit which has meant only some messages have been getting through... Again I apologise as I'm probably not the best person to help solve your issue seeing as I've no experience in the exim side of things.
