yes, you have to watch for them. However when using high threshold and low
scores, outage of blacklist is not relevant, unless some of them start
responding wiht "blacklisted" for all requests in order to stop eing used.
(this happened multiple times in history)
should this happen with any of the list scored over threshold, you'd have a
problem. Also note that blacklists may require subscription (or at least
their provider's agreement)

if you mean "sendmail", no, it does not. there actually could exist milter
that provides such functionality for sendmai.
If i understand it correctly this feature allows to stop email from being
delivered before it gets through MTA.
