Are you seeing any net tests being rul at all? if not, you need to make sure that you have net tests activated, and that SA is happy with the version of Net::DNS (and a few other things) that re required.
If you are seeing SURBL working most of the time but not all, it sounds like
you may have net connectivity problems, and the connections are timing out.
