The text in the JIRA registration attempt looked like a code style
complaint, but it's a stack overflow:

It happens when adding a page after that page has an annotation widget
that links back to the page. Normally people add the page to the
document immediately after creating the object.
We should either improve the documentation or detect that a page is
purely in memory (not sure if possible at all) or add a check for "P"
like we already have a check for "PARENT" in
COSDictionary.getIndirectObjectKeys().
