Your permit_mynetworks & check_client_access precedes your check_policy_service, policyd will not see mail if either of these is resulting in OK. Thats more than likely your issue.
To test, put check_policy_service first on your smtpd_recip... line

I just tested it but it does not work.
I may be wrong but shouldn't it have to do something with "smtpd_end_of_data_restrictions" as the error is that policies with END-OF-MESSAGE are not treated correctly?
