Cool thanks. I appear to have fixed it by changing my default policy
source to "any,!$*" to make sure authenticated users are ignored.

The answer is to ignore the misleading suggestions that suggest you can
have multiple matching policies and the highest priority one will "win".
It varies by module - some work like that, I believe quotas don't. I've
been through this one as well.

The simplest way to do things is to arrange your policy matches so that
all policies are mutually exclusive and only one will ever match.
