Have you considered something like a Kensington Expert Mouse Wireless Trackball? It isn't one of those trackball mice that you use your thumb. It has a giant trackball that you use your index finger to manipulate. I had wrist issues and moved to that a few years ago. It even works much better than a mouse since I have multiple screens and moving between them is a breeze. No more lifting the mouse.
