Follow-up to my own post in case it helps others in the future: the Debian package is broken. It uses an old version of the source files for sqlbox, which generates the above error. My solution was to rebuild the Debian packages using the original kannel source files - specifically gw/sqlbox_sql.c and gw/sqlbox-cfg.def But, this is definitely a Debian package problem, not a kannel problem.
