Well, two things jump out from your list of modules. Firstly, there's this:
> mpm_itk_module (static) Does the problem occur if you use prefork instead? Secondly, there's this: > cgi_module (shared) > cgid_module (shared) Since I'm unfamiliar with mpm_itk, it may not be a problem there but with a core MPM this would be odd.
