The steps I followed is the culmination of all the information I could find
on the Internet.  If I'm missing something critical, or have left a step
out, or need to do something additional, please offer any advise you can as
I totally bamboozled at this point.

You need to put a network sniffer on the line and see what is actually
being sent back and forth.  For example, this might be a case where
the ProxyBadHeader directive is needed.
