It sounds like you may have unwittingly edited the paragraph style to add the 
box, and now all paragraphs with that same style have a box.  Take a look at 
your paragraph styles?  If that’s the case, Libreoffice didn’t malfunction, but 
complied with what you may have inadvertently done.
