I followed Mr. Herr's instructions of removing page numbers and tabs
from ALL levels of the TOC.
That finally did the trick. Now I have a TOC with entries, but no
leading dots and page numbers,
just like Amazon wants it.

However, I am not sure about the OP's need for hyperlinks in the TOC.
But he seems satisfied.

Thomas,
I can tell you how to remove the page number, but I have no idea
how to insert a link in its place.
To remove the page number from the TOC:
1. Right click on the TOC with the unwanted page numbers and select
the edit option.
2. In the dialog, select the "Entries" tab at the top.
3. In the "Structure" line, there is a [ # ] item (for Page number)
on the right end. Select this item and press Delete.
4. Click OK.
5. Right click on the TOC again and select "Update index/table".
6. The page numbers should disappear. (I say "should", because I
have never actually needed to do this.)
Note that there is a "Hyperlink" button on the "Entries" tab which
may be what you want. Maybe someone else on this forum can help
you with that. Note also, that you may wish to remove the hanging
tab [ T ] on the right end now, which would remove the ellipses
("....") on the end of the TOC entry.
