It is real messy in google docs as well. You can download the pdf and
edit it in draw.

Hi
I was able to open the file in Abiword and view it. In Abiword it
displayed 8 pages and looked correct. I saved the file as html. Viewing
the html file I noticed that it appeared to have a large portion of the
information outside the <body></body> tags. Also, there appeared to be
several separate tables in the form; including outside the body tags.
I can forward the html file off-list if you wish to see.
