As long as the state is not ready, it will not show up in the list when you want to create the instance. Adding it directly on the secondary storage by yourself will definitely not work, the system will do this work for you. Start digging the management servers logs (/var/log/cloudstack/mangement/), to find out why the ISO isn't correctly added.
