I'd add the action context cleanup filter; it may be
cargo-cult programming at this point but in the past
it seemed necessary for file uploads to work. The
discussions regarding it are in the archives.

Other than that, I'm not sure what else to tell you
w/o seeing the code. Is the button label "Import"?
That makes me suspicious if that's also what's being
copied into the temp file (if it is "Import" you can
do a quick sanity check by making it something else
like "Fluffernutter" and seeing if that ends up being
the file's contents).
