This will happen if you click '+' immediately after entering the 'users' screen, I think a jira should be raised, however, if you follow:

1. Login: Dashboard will be displayed.
2. Administration --> USERS : The users list screen will be displayed
3. click on any of the users: That user information will be displayed.
4. click on the '+' icon: a new and empty record will be created.
5. Fill in information and click 'save' icon.
