What version of Guacamole are you using?  If you're using 0.9.13-incubating
(the release from the website) or earlier, you might want to try using the
.guacamole folder in the Tomcat user's home directory for the configuration
and the extensions folder.

Also, there are several configuration parameters required in the
guacamole.properties file that you need to fill out for Duo.  Here's the
manual page:

Can you confirm that you've configured these items in the
guacamole.properties file?  For log files, depending on how you've
installed Tomcat, the log file may be catalina.out, catalina.log, and/or
your /var/log/messages or journalctl (syslog).  Look for messages where you
see the guacamole Tomcat deployment message.
