Did you already try doing what the error message indicates you should try?
Is there anything in the logs on the 3 cassandra boxes listed (192.168.198.168, 192.168.198.169, 192.168.198.75) that indicates they had problems at that time, perhaps GCInspector or StatusLogger messages about pauses, or any drops in network utilization to indicate a networking problem?
