FWIW, for me the advantage of homebrew indexes is that they can be a lot more sophisticated than the standard -- I can hash combinations of column values to whatever I want. I also put counters on column values in the index, so there is lots of functionality. Of course, I can do it because my data becomes read-only, I know it's a luxury.
