Rich, EE is used, so transactional activation should prevent from that scenario 
ever occurring, as mentioned earlier in this thread.

typically becomes your SPOF (single point of failure), so I find it better to 
use a rock-solid hardware device here, rather than mess around with Linux and 
Apache, but you can do that too."

You can also use multiple loadbalancers and use virtual public IP addresses on 
them, so you don't have a single point of failure. That's what we use (HAProxy 
with keepalived in our case).
