under Windows i am using IconWorkshop from Axialis. 
http://www.axialis.com/iconworkshop/
IconWorkshop creates icons for Windows, Mac OS X, iOS and Android
The Pro (commercial) license costs 69€ at the moment (normally 99) and comes 
with a lifetime license.

There is also a Home version available, but that may not be used for companies.
