I can't think of anything you haven't already tried, but I'm pretty sure
the problem is in the profiles. Maybe you should just start over. Delete
everything related to Apple development in your keychain and in XCode.
Generate new certificates in the developer portal, download and import
them. Then make a new standalone with the new profile.
I know that's a lot of work but it's all I can think of.
