Bit more investigation shows that the disappearing titlebars are in fact
caused by a window manager problem.  Going to appearance->visual effects
and changing the effect level restores the title bars.

Titlebars disappear after every restart, but the graphics corruption
only appears sometimes.  note: it did *not* appear on the restart that I
provided the logs for.

While testing suspend resume, I had a complete lockup which has been
filed by apport as bug 430676
