This is Ubuntu 11.04, with 2.6.38-11-generic on a Dell Latitude E6320 and I am still seeing this issue. My external monitor (DVI through a docking station) stays off during reboot or resume, sometimes with the login screen not visible so that I have to type in the password blindly. After login, it still stays off until I do System Settings - Monitors, set the external monitor to off, apply settings and then restore the original settings.
