I believe it is possible with Outlook and Exchange but it's just a pain to setup, I've generally just left Out Of Office turned off. IIRC there is an option to specify what recipients don't get the Out Of Office message. Other option would be to setup some server side rules to send a message to whoever send you an e-mail but have some exclusions in the list.
