This bug was filed today against r237 on mako: https://bugs.launchpad.net/touch-preview-images/+bug/1293797 "nexus 4 "COMMAND ERROR" when updating apps" I too saw it, but had messed around a bit with my image to test some other things and wanted to for the next promoted image before reporting it. For some reason I want to say it also happened with the previous promoted image, but I'm not sure.
