Aha! In trying to repeat the bug I discovered a condition. It only seems to occur if a pdf is already open in another workspace. I didn't notice this before because I always have a pdf or two open for reference in another workspace.
So: Open a pdf file. In Firefox _in another workspace_: Click download link to pdf file, select open pdf from popup menu, pdf pops up and is readable but does not grab focus (light window bar).
