My problem appears to be related to pulseaudio...

The installation is a dist-upgrade from Dapper, there may be something
missing in my configuration that a clean Hardy install has set up.

The workaround for my problem was to go to:

System -> Preferences -> Sound

and change all the playback sinks from "PulseAudio Sound Server" to
"ALSA".

I will attach this to the pulseaudio project, if one exists.

I will also attach those log files (xsession-errors etc) next time I'm
on the affected machine. If I can give any other information about the
files and settings on my machine to help track this down, please let me
know.
