Try nudging the volume for Master and PCM by just a little bit. Make sure the volume controls change, this seems to force the volume to be synced to hw. If that doesn't work, try nudging all volume controls. Somehow, on my Thinkpad, I have to do this in 30% of the cases after a suspend.
